Or, you may fax the completed form to: 425-744-4571 and send payment later. We will process your order and ship as soon as payment arrives. We accept personal checks for orders up to $200. For orders over $200 we require certified checks. IF YOU USE A CREDIT CARD be sure to fill in all required information on this page of the form.
